Farm Operator Status Inquiries

When filing a Corn Loss Proof of Claim and the FSA-578 Farm Summary and Detail Listing form does not indicate your name as the Farm Operator, you should submit a letter from the FSA office indicating you are the Farm Operator. Only the Farm Operator is eligible to submit a proof of claim form under the terms of the settlement. You may also submit insurance documentation if you do not participate in the government farm program.
