Form FSA-578 Inquiries

When filing a Corn Loss Proof of Claim, you must submit the FSA-578 Farm Summary and Detail Listing report for each farm you list on your Corn Loss Proof of Claim. A FSA-578 Producers print is not acceptable documentation. The version of the FSA-578 form that includes Farm Summary and Detail Listings can be generated at an FSA office. It is not necessary to submit your original signed FSA-578 form.
